Skilled Nursing
We have skilled nursed who provide safe and effective skilled nursing care for out patients to promote their health. Under the direction of the physician, our nurses perform measures that also prevent other diseases and rehabilitate existing conditions. While providing treatment we will consult with your physician on a regular basis, and provide updates on your progress. Communication is key and so our nurses educate patients and/or their caregivers on the disease or injury process, medications, treatment plans, diet, wound care and more.
Registered Nurse.
Provides an in-home assessment of patient needs and develops a proper plan of care to suit them in consultation with physician’s orders
Licensed Vocational Nurse.
Coordinates with interdisciplinary team, patient/family and referring agency in order to provide skilled nursing care
Medical Social Worker
As part of our health care interdisciplinary team, our Medical Social Workers (MSW) help meet our patient’s needs by preparing them for life out side a residential medical setting. MSWs work with our patients to assess the full range of their needs and help them access services and resources to improve their quality of life.

In many ways, social workers are considered vital in helping our patients get access to important resources. They’re excellent in orchestrating the coordination or patient needs to help them reach their goals.
Therapy Services
We have expert therapists in the North Texas area ready to assist you through rehabilitation. Being state licensed, our therapists evaluate and design personalized treatment plans for our patients. We work alongside physician’s orders when we send out therapy for an in-home assessment. From there a plan of care is created and followed. Communication is maintained between therapy, nursing team, and primary physician, ensuring that the best care is given to our patient.
Physical Therapy.
Therapy focuses on helping patients return previous functional activity by helping them regain strength, mobility, physical function and independence with daily activities
Occupational Therapy.
Therapy is a rehabilitative service to help patients who are recovering from injury or experiencing physical & cognitive changes in skills needed for daily activity
Speech Therapy.
Therapy helps patients improve speech, memory and swallowing disorders. Speech Language Pathologists educate the patient & family regarding the diagnosis along with teaching ways to improve communication
Home Health Aide
We have home health aides who provide direct patient care under the supervision of our registered nurses. Our aides are CNA certified and skilled in providing assistive care to out patients. Their high quality training and excellent care provides great comfort to our patients who require partial or complete assistance.
Home Health Aide.
Care is provided under supervision of our registered nurses. This includes but is not limited to patient condition monitoring, assistance with activities of daily living, and/or minor housekeeping pertaining to our patient.
Respite.
Providing short term relief for primary caregivers, we can arrange for our aides to give the necessary care for our patient
